Established in 2001, Unity Hospice Care is a full service Hospice care provider. We are committed to providing compassionate care to our patients and families. We believe that each individual should be granted the opportunity to die in a dignified and comfortable manner. Our focus is to enable our patients to live as fully and comfortably as possible while providing spiritual and palliative care through God’s grace.

This special care is primarily provided in the patient’s home. However, those living in nursing homes, assisted living facilities or other in-patient facilities may also be eligible.

Our goal is to ensure that their last moments are comfortable, that their departure from this life is peaceful and that you can rest easy knowing you did all you could for them in their final hours.

Honoring Our Veterans Unity Hospice Care is proud partners with the We Honor Veterans Program.

Hospice care is available for veterans who have terminal illness or disease with a diagnosis of six months or less to live. At Unity Hospice Care, our team will work with you, your family, and your physician.

Hospice Care is part of the VHA Stand Medical Benefits Package. All enrolled Veterans are eligible if they meet the clinical need for hospice. Enrollment information is available on the Veterans Affair Health Benefits website. There are no Copays for Hospice Care whether it is provided by the VA or with Unity Hospice Care.

Grief Support

Unity Hospice Care offers Grief Support to those who have experienced the death of a loved one. Grief is a reaction to a loss affecting all parts of our lives physically, emotionally, spiritually, socially, and psychologically. It is a very personal journey to which there are no roadmaps, guidelines, or specifics. Therefore, it is our belief that participation in a grief support group is one of the most effective ways to cope with the pain associated with the loss of someone you love.

If you are interested in participating in a group or wish to speak with someone on an individual basis, our bereavement team will be available to assist you.
